ABSTRACT

Objective:To explore the mechanism of the prescription consisting Aconiti Lateralis Radix Praeparata and Epimedii Folium in the treatment of chronic heart failure (CHF) based on network pharmacology,followed by verification in H9c2 myocardial cells with hypoxia-reoxygenation injury <italic>in vitro</italic> and in zebrafish with vascular endothelial growth factor receptor (VEGFR) tyrosine kinase inhibitor Ⅱ (VRI) -induced vascular insufficiency. Method:The active ingredients in Aconiti Lateralis Radix Praeparata and Epimedii Folium were searched from the Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform (TCMSP),the corresponding target genes from the Universal Protein Resource (UniProt), and the CHF-related targets from Online Mendelian Inheritance in Man (OMIM) and GeneCards. Both the active ingredient-potential target network and the active ingredient-CHF-related target network were generated using Cytoscape 3.6.1, followed by the protein-protein interaction (PPI) network construction and Gene Ontology (GO) and Kyoto Encyclopedia of Genes and Genome (KEGG) enrichment analysis based on MetaScape. H9c2 myocardial cells exposed to hypoxia-reoxygenation were selected for determining the proliferation-promoting effect by methyl thiazolyl tetrazolium (MTT) assay. The protein expression of B-cell lymphoma-2(Bcl-2), Bcl-2-associated X protein(Bax),cysteinyl aspartate-specific protease-3(Caspase-3), protein kinase B(PKB/Akt),phosphorylated protein kinase B(p-Akt),phosphorylated extracellular signal-regulated kinases 1/2 (p-ERK1/2),extracellular signal-regulated kinase 1/2 (ERK1/2), and poly adenosine diphosphate ribose polymerase(PARP)was detected by Western blotting. The efficacy of the prescription in promoting angiogenesis was verified in a zebrafish model of VRI-induced vascular injury. Result:There were 28 active ingredients for the prescription, 209 corresponding targets, 1 296 CHF-related targets, and 94 common gene targets shared by the prescription and CHF. PPI network clustering suggested that Aconiti Lateralis Radix Praeparata and Epimedii Folium alleviated CHF by interfering with cell differentiation and metabolism and angiogenesis. GO analysis revealed that CHF relief was achieved via the intervention in such biological processes as cell migration,vascular development, and angiogenesis. Pharmacodynamic experiments verified that Epimedii Folium (10 mg·L<sup>-1</sup>) alone and the prescription (10 mg·L<sup>-1</sup>)both enhanced the proliferation of H9c2 myocardial cells under the hypoxia-reoxygenation condition (<italic>P</italic><0.05),while the latter also increased the expression of Bcl-2,Bcl-2/Bax, and PARP (<italic>P</italic><0.05) and reduced the expression of Caspase-3, Akt, and ERK (<italic>P</italic><0.05). The prescription at the concentrations of 0.3 and 0.1 g·L<sup>-1</sup> promoted angiogenesis (<italic>P</italic><0.05). Conclusion:Aconiti Lateralis Radix Praeparata and Epimedii Folium exert the therapeutic effect against CHF via multiple ingredients,multiple targets, and multiple channels. Such combination promotes the proliferation of H9c2 myocardial cells under hypoxic condition and protects zebrafish from vascular injury by up-regulating the expression of Bcl-2 and PARP,increasing Bcl-2/Bax ratio,and down-regulating the expression of Caspase-3,Akt, and ERK.

ABSTRACT

Objective To explore the therapeutic effect of group medical visits on high-risk elder people suffering senile osteoporosis.Methods A total of 86 high-risk elder people aged over 65, suffering senile osteoporosis and with the OSTA index below-4 were randomly selected, and were separated into an intervention group and a control group, with 43 patients belonging to each group.The dual-energy X-ray absorptiometry(DXA) was used to measure the left femoral neck bone mineral density (LFN BMD), and relevant biochemical indexes thereof and biochemical indicators of bone metabolism thereof were measured at the same time.The osteoporotic knowledge test(OKT) and the osteoporotic self-efficiency scheme(OSES) were used to make relevant evaluations.Group interventions included the following four items: health edutaion and individual technical guidance, state of illness monitoring, group activities and prescription of medicines.At one year after the intervention, the above-mentioned items were to be evaluated again for the two groups.Results After the intervention, the blood calcium(Ca2+), phosphorus(P3+) and LFN BMD of the two groups had no statistical difference(P>0.05);the OKT and OSES scores as well as the level of 25(OH)VitD3 of the intervention group were all higher than those of the control group as well as those of the intervention group before the intervention(P<0.05).After intervention, the PTH, PINP and β-CTX levels of the intervention group were lower than those of the control group as well as those of the intervention group before the intervention(P<0.05).Conclusion The group medical visit is an effective community-based management intervention mode suitable for high-risk patients suffering osteoporosis.

ABSTRACT

[ Objective] To investigate the influence factors of body mass index ( BMI ) in elderly in Shanghai Dapuqiao community , thus providing a basis for further management and treatment of chronic diseases and health education in this regard . [ Methods] Physical examination data were analyzed on a series of 2,106 elderly people over 65 by means of multiple stepwise regression in relation to relative factors influencing IBM in the elderly . [ Results] Among the 2,106 elderly people,624 had various physical indicators normal and no previous history of chronic diseases , accounting for 29 .6%.As to the anomaly detection rates,the top five were in triglyceride(TG)(65.8%),waistline(63.9%),BMI(60.3%),blood pressure(BP)(52.5%),and total cholesterol(TC)(31.7%).Women were found to be higher than men in values of BMI,TC,TG,HDL-C,LDL-C,SBP.But men were higher than women in values of wasitline,UA and DBP.The positive detection results in different age groups were different with TG ,LDL-C,UA.Through multiple stepwise regression analysis ,BMI was found to be in positive correlation with sex ,wasitline,TC, LDL-C,UA,and in negative correlation with age and HDL-C. [ Conclusion] Hyperlipidemia, high blood pressure ,overweight or obese were higher in detection rate among the elderly in Dapuqiao community . BMI was in relation with waistline , sex , blood lipid , age and UA .To investigate the basic health status of elderly people over 65 is important , which is of benefit to preventing and treating chronic disease in the community.

ABSTRACT

To investigate the efficacy and safety in different doses of atorvastatin for elderly hyperlipidemia . [ Methods] A total of 153 elderly patients with hyperlipidemia were included , who were randomly divided into two groups .Group A ( n=79 ) received 10 mg/d atorvastatin , and group B ( n=74) 20 mg/d.Before and after treatment of 2 weeks, 8 weeks, were checked the level of blood lipid , functions of liver and kidney, creatine kinase, creatine kinase isoenzymes. [Results] After treatment, the level of TC (cholesterol) was reduced by 19.6%(A) and 20.0%(B) respectively in the two groups. LDL-C (low density lipoprotein cholesterol ) decreased by 17.4%(A) and 20.1%(B) respectively in the two groups.The level of TC, LDL-C in the two groups showed no difference after treatment (P<0.01).The medicine was not found to cause adverse effect in liver and kidney function and myolysis in the two groups (P>0.05).After taking medicine,The level of uric acid(UA) was decreased in group A (P<0.01). There was no difference in the two groups (P>0.05). [Conclusion] Oral administration of lower do-ses of atorvastatin not only lowers the lipid in elderly patients , but also lowers the blood uric acid .And ad-ministration of lower and higher doses of atorvastatin are both effective and safe for elderly hyperlipidemia .
